Mesothelioma and Rare Peritoneal Tumors

Information about primary peritoneal tumors and the importance of treatment at specialized centers.

Introduction

In this section we discuss tumors that originate in the peritoneum itself (not metastases from other organs). These are rare tumors that require diagnosis and treatment at specialized centers.

Peritoneal Mesothelioma

What is it?

Peritoneal mesothelioma is a tumor that originates in the cells that line the peritoneum (mesothelial cells). It is related to asbestos exposure in some cases, although it can also occur without this risk factor.

Symptoms

  • Abdominal distension
  • Abdominal pain
  • Ascites (fluid in the abdomen)
  • Weight loss
  • Fatigue

Treatment

In selected patients, treatment combines:

  • Cytoreductive surgery: Removal of visible tumor
  • HIPEC: With specific drugs for mesothelioma
  • Systemic chemotherapy: In some cases

Results have improved significantly in recent years in specialized centers.

DSRCT (Desmoplastic Small Round Cell Tumor)

What is it?

DSRCT is a rare and aggressive tumor that mainly affects young people. It usually presents with multiple implants in the peritoneum.

Treatment

Treatment is complex and usually involves:

  • Intensive chemotherapy
  • Cytoreductive surgery in selected cases
  • HIPEC in some centers
  • Radiotherapy as consolidation

It is essential to be treated at centers with experience in this disease.

Other rare peritoneal tumors

  • Primary peritoneal carcinoma: Similar to ovarian cancer, occurs without obvious ovarian tumor
  • Peritoneal serous carcinoma: Variant of primary peritoneal tumor
  • Solitary fibrous tumor: Usually benign, but may recur
  • Peritoneal leiomyosarcoma: Rare sarcoma of the peritoneum

Importance of specialized centers

Rare tumors require:

  • Accurate diagnosis: Pathological study by experienced specialists
  • Multidisciplinary discussion: Each case requires careful evaluation
  • Experienced surgical team: In complex cytoreductive surgery
  • Specific protocols: Tailored to each type of tumor

The value of a second opinion

In rare tumors, seeking a second opinion is especially important:

  • Diagnosis may be difficult and require pathology review
  • Treatment options may be different at specialized centers
  • Experience in specific disease matters
  • Access to clinical trials
